During the inspection, the inspector and the administrator reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

Inspector Notes

An unannounced, on-site focused inspection was initiated and completed on 04/07/2025. The on-site inspection began at 9:30 and ended at 11:15 The inspector and the administrator reviewed compliance in the areas listed above. There were 19 children present and 4 staff. The inspector reviewed 1 children?s record and 5 staff records on-site. This inspection included document review, tour of the facility, interviews, and observations.

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

SYSTEMIC VIOLATION
Physical examinations completed by a physician shall be obtained prior to the first day of attendance or within 30 days after the first day of attendance.

There was no physical examination in the record of child 1, who had been enrolled for six months.
Plan of Correction: Provider will obtain from parent.
Standard 8VAC20-780-160-A
SYSTEMIC VIOLATION
Documentation of a negative tuberculosis (TB) screening must be submitted at the time of employment, before coming into contact with children, and shall have been completed within the last 30 days of the date of employment.

Staff 5 TB screening was dated 9 months prior to date of employment.
Plan of Correction: Moving forward provider will ensure TB?s requested before hire have been completed within the last 30 days.

Staff shall provide awake infants not playing on the floor or ground a change in play space at least every 30 minutes or more often as determined by the individual infant?s needs.

Child 1 (age 8 months) was observed awake in a play yard for approximately 58 minutes.
Plan of Correction: Staff will ensure children will have a change in play space at least every 30 minutes.

Documentation of at least 2 references to character, reputation, and competency are to be checked prior to employment.

Staff 5, who has been employed one week, did not have any completed reference verifications on file.
Plan of Correction: Provider will obtain references.
