Inspector Notes

An unannounced 60-day monitoring inspection was conducted on 6/4/24 from 9:40am to 10:55am with the provider and one new assistant. There were six children in care, ranging in age from 13-months to three-years-old, for a total of 17 points. Children were observed having free play, reading and discussing a book about fruits, washing hands, eating a snack, and doing puzzles. Five child records and one assistant record were reviewed. The provider and assistant have current certification in CPR and First Aid. The provider is not administering medication at this time, and is aware of the requirement to obtain Medication Administration Training before changing their policy. Morning snack served was apples, crackers, and milk. The emergency drill log, attendance, and menu were also reviewed. Please complete the "Plan of Correction" and "Date to be Corrected" areas on the Violation Notice for each violation cited and return to me by close of business on 6/12/24. Plans of correction should include steps to correct the noncompliance with the standard, and measures to prevent the noncompliance from occurring again.

Based on review of five child records, the provider did not obtain documentation of a physical examination for each child within 30 days after the first day of attendance. Evidence: The record for Child D, with a start date of 4/8/24, did not contain documentation of a physical.
Plan of Correction: Physical was misplaced and has been found and returned to their folder.
Standard 8VAC20-800-60-B
REPEAT VIOLATION Based on review of five child records, the provider did not obtain all of the required documentation for child records. Evidence: 1. The records for Child A, Child B, and Child C did not contain parents signed acknowledgement of the receipt of all of the required policies and procedures. 2. The record for Child A did not contain the name, address, and phone number of a second designated person to contact in case of emergency if the parent cannot be reached. The record for Child D did not contain the addresses of two designated people to contact in case of emergency if the parent cannot be reached.
Plan of Correction: Paperwork will be revisited and parents will be asked to signed needed paperwork.
