Inspector Notes

An unannounced monitoring inspection was initiated on 7/24/2024 and completed on 8/8/2024 in response to a self-report received by the licensing office on 7/18/2024 relating to Supervision of Children. The inspector reviewed 2 children's records and 1 staff record on-site. This inspection included document review, tour of the facility, interviews, and observations.

The preponderance of evidence gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Please complete the plan of correction (POC) and date to be corrected for each violation cited on the violation notice. Specify how the deficient practice will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on 8/15/2024. A POC submitted after this date will not appear on the public website.

Standard 8VAC20-780-340-A
Based on interviews, photographs and center documentation, the center did not ensure that when staff are supervising children, they shall always ensure their care, protection and guidance.

On 7/18/2024 at approximately 8:30 am, staff #1 exited the classroom leaving 8 two-year-old children alone.
During this time, child #1 sustained 7 bites to the face, arm and back which resulted in outside medical treatment.
Plan of Correction: "Had all staff do training on supervision of children, reviewed and stressed how important sight and sound is."
Standard 8VAC20-780-340-F
Based on interviews and center documentation, the center did not ensure that children under 10 years of age shall always be within actual sight and sound supervision of staff.

Evidence: On 7/18/2024 at approximately 8:30am, staff #1 exited the classroom leaving 8 two-year-old children alone and out of sight and supervision of staff.
Plan of Correction: "Spoke with teacher to make sure she understands how important sight and sound of children is."

Based on interview and center documentation, the licensee did not ensure that the center's activities, services, and facilities are maintained in compliance with these standards, the center's own policies and procedures that are required by these standards, and the terms of the current license issued by the department.
Evidence:
The center's supervision policy, documented in their staff handbook, states that "Children must be within your sight and sound supervision at all times".

On 7/18/2024 at approximately 8:30 am, staff #1 exited the classroom leaving 8 two-year-old children out of sight and sound supervision of staff.
