Inspector Notes

An unannounced, on-site monitoring inspection was initiated and completed on, as a 7/14/2025. The on-site inspection began at 12:10 pm and ended at 3:20 pm. The inspector reviewed compliance in the areas listed above. There were 40 children present and 7 staff. The inspector reviewed 5 children?s records and 5 staff records on-site] on 7/14/2025. This inspection included document review, tour of the facility, interviews, observations, measurements.

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program

Standard 8VAC20-780-60-A
The center is required to maintain and keep at the center a separate record for each child enrolled which shall contain all required information.
The record for child #5, who has been enrolled int he program for over 2 months did not contain the following required information: the address of one parent ; place of employment of the other parent, allergies and intolerance to medication or any other substances, and actions to take in an emergency situation, chronic physical problems and pertinent developmental information and any special accommodations needed; previous child day care and schools attended by the child; any additional programs or schools that the child is concurrently attending and the grade or class level.
Plan of Correction: The parent will be asked to complete this missing information on the enrollment form by Monday. The program director will review applications at the time of enrollment and ensure that there are no spaces left blank.
