Inspector Notes

An unannounced monitoring inspection wascompleted on 04/11/2025, as a part of the licensure period. The on-site inspection began at 1:30pmand ended at 2:45pm.The inspector reviewed compliance in the areas listed above. This inspection included: document review, tour of the facility, interviews, observations and measurements. There were 18 children present and 2 staff. during nap. The inspector reviewed 2 children?s records and 2 staff records. Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Standard 22.1-289.035-B-2
All staff are required to submit fingerprinting prior to employment. Staff B (hired 12/2024) did not have the results of a fingerprint prior to her employment at the center.
Plan of Correction: The center had a copy of the staff member's fingerprints, however they were done by another entity. The director has contacted Office of Background Check to see if the fingerprints are eligible for portability. To apply for the portability it is a process because the center has to register with OBI. The staff went to obtain fingerprints via field print on 04/14/2025.
Standard 8VAC20-780-520-C
If diaper ointment or cream is used it shall be in the original container and labeled with the child's name; shall be inaccessible to children; and a record shall be kept that includes the child's name, date of use, frequency of application and any adverse reactions. There was a tube of diaper ointment on a low shelf in reach of children. It was not labeled to identify which child it belonged to and the center did not have a written record of application.
Plan of Correction: The diaper cream was moved during the inspection so that it was inaccessible to children. The diaper cream was labeled with the child's name and the center will begin documenting application.

When centers choose to provide meals or snacks the posted menu should list any substituted food. April's meals and snack menu did not list that coleslaw was substituted on all of the days it listed carrots as a lunch component.
Plan of Correction: This was an oversight. The menu will be updated to reflect any changes made to the menu
Standard 8VAC20-780-80-A
For each group of children, the center shall maintain a written record of daily
attendance that documents the arrival and departure of each child in care as it
occurs. The center did not have a written record of daily attendance that documents the arrival and departure of 2 of the 18 children present at time of inspection.
Plan of Correction: The director will ensure the center keeps an accurate account of the children's attendance that includes arrival and departure time.
