Inspector Notes

An announced initial inspection was initiated on 1/25/24 and concluded on 1/26/24. The provider intends to provide care for up to 104 children, ages six weeks to 12 years old. At the time of the inspection there were no children in care. The classrooms, kitchen, bathrooms, and playground were toured. The center does not intend to administer medication or provide transportation at this time. Records reviewed included four children?s and five staff records. Practice drill plans, emergency supplies and other required records and postings were also reviewed. The information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice and discussed during the exit interview.
A six-month conditional license is recommended to allow the applicant the opportunity to demonstrate continued compliance with the Standards for Licensed of Child Day Center. All future inspections will be unannounced. The first unannounced inspection will be conducted within 60 days from the date the license is issued. Based on observation and interviews, it was determined that the facility did not ensure all areas and equipment of the center shall be maintained in a safe and operable condition
Evidence:
1. In the four-year-old classroom there was a broken dress up play unit. The unit was missing screws at the base. The board at the base was not secure and the unit was unstable when touched.
2. There was a metal pipe approximately an inch in diameter protruding out about 4? from a brick wall in the fenced playground area. The location and height (approximately 2 ? feet from the ground) of the pipe posed an impalement hazard. There was a child size table next to the pipe.
3.The Director confirmed the equipment above was broken or protruding.
Plan of Correction: The Director removed the broken dress up unit on 1/25/2024. The Director installed a cover over the pipe on the brick wall.
