Inspector Notes

An unannounced 60- day monitoring inspection was conducted on April 15, 2024, from approximately 11:40 AM until 2:10 PM. There were 77 children present in the center, ranging in ages from two months to five years of age, with 9 staff supervising. Children were observed napping. The inspector reviewed compliance in the areas of administration, staffing and supervision, programming, staff qualifications and training, special care and emergencies. A total of seven child records and seven staff records were reviewed.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Please complete the plan of correction and the date to be corrected for each violation listed on the violation notice and return it to the department within 5 business days from receipt. Your plan of correction should contain: 1) steps to correct the noncompliance with the standard(s); 2) measures to prevent the noncompliance from occurring again; and 3) person(s) responsible for implementing each step and/or monitoring any preventative measures.

Based on a review of seven (7) staff records, the center failed to obtain a central registry finding within 30 days of employment.

Based on record review, the center did not ensure to that each child shall have a physical examination by or under the direction of a physician within 30 days after the first day of attendance.

Based on record review, the center did not ensure that each staff member shall submit documentation of a negative tuberculosis screening within the last 30 calendar days of the date of employment and be signed by a physician, physician's designee, or an official of the local health department.
