Inspector Notes

A Subsidy Health and Safety inspection was conducted on 7/23/2024 to review supplemental health and safety requirements. There were 157 children present with 25 staff supervising.

A licensing inspection was conducted on the same date as the SHSI supplemental inspection; the two inspections combined cover all subsidy health and safety requirements. Only subsidy requirements that are not covered in licensing standards are reviewed during this supplemental inspection.

Violations were found during the supplemental subsidy health and safety inspection requirements and are identified on the violation notice.

Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the deficient practice will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on 07/31/2024. A POC submitted after this date will not appear on the public website.

Based on a review of 10 staff records and interview, the vendor did not ensure that one staff had a current certification in cardiopulmonary resuscitation (CPR) appropriate to the ages of children in care.

Evidence:
1. The record of staff # 10 (date of employment: 2/23/2024) contained a certification that expired on 6/23/2024.
2. The vendor acknowledged that staff #10 did not have a current certification.
