During the inspection, the inspector reviewed the areas listed above. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.
Inspector Notes
An unannounced inspection was initiated on 3/21/2025 and completed on 4/8/2025 in response to a complaint received by the licensing office on 3/14/2025 relating to allegations in the areas of care and protection and injury reporting. The inspector reviewed 1 child?s record on site. This inspection included document review, interviews, and observations.
The preponderance of evidence gathered during the investigation supports one of the allegations; therefore, the complaint is determined to be valid. Information gathered during the inspection determined non-compliance with applicable standards or law, and the violations are documented on the violation notice issued to the program.
Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five business days from today, which will be the close of business on 4/15/2025. A POC submitted after this date will not appear on the public website.
Violations
11. On 3/13/2025 at approximately 4:27 pm, child #1 tripped and struck his forehead on a paved area of the playground. A quarter sized hematoma appeared within a few minutes. Child #1 was immediately brought inside for an evaluation by management and was returned to the playground within an approximate 5 minute window to sit with ice. Three witnesses stated that shortly after the head injury, child #1 demonstrated signs that were compatible with a serious head injury to include stumbling while walking and not being as verbally responsive when spoken to. Injury report documentation indicated that child #1's parent was notified via phone call at 5:05 pm, approximately 38 minutes from the time of injury.
2. Child #1 was taken to a medical facility and diagnosed with a concussion.
