During the inspection, the inspector reviewed the areas listed above. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

Inspector Notes

An unannounced inspection was initiated on 04/03/2025 and completed on 04/24/2025 in response to a complaint received by the licensing office on 04/02/2025 relating to allegations of supervision and illness. The inspector reviewed one child?s records and three staff records on-site. This inspection included document review, tour of the facility, interviews, and observations.

The preponderance of evidence gathered during the investigation supports the allegation; therefore, the complaint is determined to be valid. Information gathered during the inspection determined non-compliance with applicable standards or law and the violations are documented on the violation notice issued to the program.

Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on 05/05/2025. A POC submitted after this date will not appear on the public website.

Children under 10 years of age always shall be within actual sight and sound supervision of staff.

Based on video surveillance, on 04/01/2025, child #1, age 3 years, entered classroom 3C from the playground and was unsupervised for approximately one minute and seven seconds. Child #1 then exited the classroom to return to the playground to greet their parent. Parent returned to the classroom with child #1.

On 04/03/2025, staff #2 and staff #3 were responsible for the supervision of 22 preschoolers on the playground. Two children in their group were playing around the corner of the building near the gated entrance of the fence and were not within sight and sound supervision of staff #2 and staff #3. Staff #2 and staff #3 were standing next to one another around the corner of the building.

Standard 8VAC20-780-40-E
The operational responsibilities of the licensee shall include ensuring that the center's activities and services are maintained in compliance with the center's own policies and procedures that are required by the standards.

The center?s policy for playground supervision states, ?Line of sight must be maintained in all areas of the playground. Remove obstacles and position staff accordingly. Staff must spread out into zones to maintain full visualization of the entire playground, and to be sure children are not exiting the playground or reentering the building on their own.?

On 04/01/2025, child #1, age 3 years, exited the playground area and entered classroom 3C unsupervised for approximately one minute and seven seconds. On 04/03/2025, two children were outside the line of sight of staff #2 and staff #3 while on the playground. Staff #2 and staff #3 were not spread out into zones to maintain full visualization of the playground.
