The inspection report was amended on 12/29/2025.
An unannounced non-mandated monitoring inspection was initiated and completed on October 21, 2025, in response to a self-report received by the licensing office on October 8, 2025 relating to supervision. The inspector reviewed 1 child?s record and 2 staff records on-site. This inspection included document review, interviews, and observations.
Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.
Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five business days from today, which will be the close of business on 12/3/2025. A POC submitted after this date will not appear on the public website.
Violations
2On October 8, 2025, at approximately 9:05am, child #1 (20-month-old), was left outside on the playground. Based on video footage reviewed on 10/21/2025, staff # 1 and staff #2 were transitioning the Toddler I classroom from the playground into the classroom when Child #1 was left outside. Child #1 was outside for approximately 3 minutes, before staff #1 and #2 realized the child wasn?t in the classroom. Staff # 2 turned around and child #1 was standing outside of the window looking into the classroom.

The center's supervision policy requires that children must be supervised by sight and
sound at all times. At no time is a teacher to leave a child unsupervised. On October 8, 2025, staff gathered the toddler I class to transition indoors from the playground. Child #1 stood outside of the classroom door for approximately 3 minutes before Staff #2 opened the door to retrieve the child.
