(Repeat Violation) When staff are supervising children, they shall always ensure their care, protection, and guidance.
On October 24, 2025, at approximately 8:30am, Child #1 (3-year-old) was left outside during transition. Based on video footage reviewed on 10/29/2025, Staff # 1 was transitioning five preschoolers from the preschool building to the school age building. During the transition, Child #1, (3-year-old), fell on the ground. Staff #1 proceeded to the school age building along with the other 5 preschoolers. Staff #1 never turned around to acknowledge Child #1. Name to face was not done once Staff #1 and the 5 preschool age children arrived at the school age building. Staff #1 admitted to not completing name to face once they arrived at the school age building. Child # 1 was outside on the ground for approximately two minutes before Staff # 2 retrieved the child and brought child #1 inside. Child #1 was not upset or crying when retrieved from outside.

The center's supervision policy states that "children must be supervised by sight and
sound at all times. At no time is a teacher to leave a child unsupervised". On October 24, 2025, Staff #1 gathered five preschool age children to transition from the preschool building to the school age building. During the transition, Child #1 (3-year-old) was seen walking along with Staff #1 and the five preschool age children. Child #1 fell on the ground and Staff #1 and the 5 preschoolers continued walking to the school age building. Child #1 was left outside alone for approximately 2 minutes before Staff #2 retrieved the child and brought Child #1 inside.
