Physical examinations completed by a physician shall be obtained prior to the first day of attendance or within 30 days after the first day of attendance.

There was no physical examination in the record of child #4 and child #8. Child #4 has been enrolled for 30 days and child #8 has been enrolled for over four months.
Plan of Correction: The Physical has been placed in each child's file.
Standard 8VAC20-780-210-A
Program leaders shall be at least 18 years of age, have fulfilled a high school program completion or the equivalent, and meet the qualifications of six months? experience and 24 hours of training within the six months before being promoted or beginning work or one month after being promoted.

The record for staff #3, # 4, #5 and #8, did not contain lead teacher qualifications.

There were no staff at the center on the day of the inspection with current daily health observation training.

Standard 8VAC20-780-270-A
The center shall ensure that areas and equipment of the center, inside and outside, are maintained in a clean, safe and operable condition.

1. A portion of a fence located on the preschool playground, has broken wood exposing sharp edges that can snag skin or clothing.
2. Numerous area rugs in multiple classrooms, showed extreme wear and tear including dirt and stains.
3. There were several areas of chipped and peeling paint in window seals located in the cruisers classroom, the chipmunk?s classroom, and the koalas classroom.
4. On the center's bus used to transport children, there were multiple seats torn with exposed stuffing and there was a hole in the back of a seat.
Plan of Correction: I will reach out to our team to get a plan in place to make all nesesary repairs.

Where playground equipment is provided, resilient surfacing shall comply with minimum safety standards and shall be under equipment with moving parts or climbing apparatus to create a fall zone free of hazardous obstacles.

The mulch located under the swings on the preschool playground, was measuring between 1 ? - 2 inches.
Plan of Correction: I will reach out to our team to order mulch.

Staff shall request at least annually, parent confirmation that the required information in the child's record is up to date.

The record for child #6 and child #9, did not have a parent?s signature for an annual review of the child's record. Child #6 has been in care for over two years and child #9 has been in care for over three years.
Plan of Correction: Updated forms are going out 1/26/2026. All families will have any updates added to their file.

Lockdown procedures shall be practiced at least annually.

There was no documentation that the annual lockdown drill had been completed in 2025.
