(Repeat Violation and Systemic Deficiency) When staff are supervising children, they shall always ensure their care, protection, and guidance.
Based on video footage reviewed on December 17, 2025, staff #1 was gathering a two-year-old class at the main entrance of the school age building. Staff# 1 was preparing to transition the children back to the preschool building. Staff #1 did not ensure that all children were accounted for prior to leaving the building. During the transition, Child #1 climbed into a "buggy? that was being stored in the hallway next to the door. Staff #1 was not aware that child #1 had climbed into the "buggy" and was left in the building. Child #1 climbed out of the buggy and walked down the hall towards the back of the building and entered into an empty classroom. When Child #1 entered back into the main hall, a parent saw the child and walked the child down to Staff #2?s classroom. Staff #2 then called admin to inform them that Child #1 was left alone in the building. Admin came to retrieve Child #1 and transitioned the child back to the preschool building. Child #1 was left unattended for approximately 4 minutes and showed no signs of distress.

In the center's handbook the supervision policy states, ?that children under 10 years of age always shall be within actual sight and sound supervision of staff?. The center?s policy also states that ?taking roll and keeping an accurate roll book are vital to the safety of the children in care. Staff shall always be able to account for the children in their care, and the office staff shall immediately be notified of lost or missing children?. On December 5, 2025, Staff #1 was gathering a two-year-old class at the main entrance of the school age building. Staff #1 was in the process of transitioning the children back to the preschool building. Staff #1 did not ensure that all children in care were accounted for prior to leaving the building. During the transition, Child #1 climbed into a "buggy? that was being stored in the hallway next to the door. Staff #1 was not aware that Child #1 had climbed into the "buggy" and was left in the building. Plan of Correction: The program will continue to comply with self-reporting requirements and will continue to monitor staff for compliance with policies and regulations. The staff member involved in this supervision incident has separated from the company and remaining staff members have been reminded of the requirement to follow Cadence Policies and VA DOE supervision requirements. Additionally, Cadence Education regional leadership is partnering with the school administrators
to increase oversight of standards during transitions to one space from another.
