During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

Inspector Notes

An unannounced inspection was initiated on February 25, 2026, and completed on March 2, 2026, in response to a complaint received by the licensing office on February 24, 2026, relating to allegations of behavior guidance, forbidden actions, and policy. The Department issued a Notice of Intent to place licensee on probation and require licensee to contact parents, guardians or other responsible persons regarding health and safety violations on 4/3/2025,10/21/2025, and 10/29/2025 as a result of high-risk violations. The inspector reviewed 2 children?s records and 1 staff record on-site. This inspection included document review, interviews, and observations.

The preponderance of evidence gathered during the investigation supports the allegation(s); therefore, the complaint is determined to be valid. Information gathered during the inspection determined non-compliance with applicable standards or law, and the violations are documented on the violation notice issued to the program.

Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five business days from today, which will be the close of business on March 11, 2026. A POC submitted after this date will not appear on the public website.

Standard 8VAC20-780-340-A
When staff are supervising children, they shall always ensure their care and protection.

(Repeat Violation and Systemic Deficiency) On 2/23/2026, Staff #1 forcefully pulled Child #1, (age two), by their right wrist and moved them approximately six feet across the floor until the child stood up on both feet. Child #1 was visibly upset and reportedly cried. On the same day, Staff #1 lifted Child #2 (age two) by the upper arms and forcibly placed the child at their eye level, in close proximity to their face.
Plan of Correction: Immediately upon notification of the incident, school administration removed the staff member from the care of children, and their employment has since been terminated. All remaining staff have been retrained on the requirements of child interactions.
Standard 8VAC20-780-400-B
Behavioral guidance shall be constructive in nature, age and stage appropriate, and shall be intended to redirect children to appropriate behavior and resolve conflicts.

Staff #1 forcefully moved Child #1 approximately six feet across the floor by their wrist and inappropriately lifted Child #2 to their face to speak to them. A witness stated that child #1 was crying during the interaction.
Plan of Correction: Immediately upon notification of the incent, school administration removed the staff member from the care of children, and their employment has since been terminated. All remaining staff have been retrained on the requirements of child interactions.

The following actions or threats thereof are forbidden: roughly handling a child.

In video footage from 2/23/2026, Staff #1 was seen roughly handling two, two-year-old children. Child #1 was pulled across the floor by a single wrist and Child #2 was lifted off of the ground by their upper arms and held close to the staff member?s face while being spoken to. A witness reported that the incident was inappropriate, and Child #1 was observed crying.
Plan of Correction: Immediately upon notification of the incent, school administration removed the staff member from the care of children, and their employment has since been terminated. All remaining staff have been retrained on the requirements of child interactions.
