During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. Inspector Notes
An unannounced, on-site focused inspection was initiated and completed on March 23, 2026, as a part of probation. The Department issued a Notice of Intent on January 22, 2026, to place the licensee on probation and require the licensee to contact parents, guardians, or other responsible persons regarding heal and safety violations as a result of high-risk violations.
The on-site inspection began at 10:15am and ended at 11:15am. The inspector reviewed compliance in the areas listed above. The inspector reviewed 1 child?s record and 1 staff record on-site. There were 101children present and 23 staff. This inspection included interviews, observations, a tour of the facility, and document review.
Information gathered during the inspection determined noncompliance with applicable standards or law, and violations are documented on the violation notice issued to the program.
Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five business days from today, which will be the close of business on April 6, 2026. A POC submitted after this date will not appear on the public website.

On March 23, 2026, Staff #1 forcefully walked Child #1, age 3, approximately three feet while holding the child?s hand above their head. Staff #1 pushed the child forward while releasing their hand and in an elevated tone, instructed them to sit on the border that surrounded the play equipment. Child #1 cried and was consoled by a witnessing staff member.
Staff #1 roughly handled Child #1, age 3, when they forcefully held them by their hand which was positioned over their head and pushed them forward. Child #1 was observed crying.
