During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.
Inspector Notes
An unannounced, on-site renewal inspection was initiated on September 30 2025 and completed on September 30 2025. The on-site inspection began at 10:00am and ended at 11:15am. The inspector reviewed compliance in the areas listed above. There were 8 children present and 3 caregivers; the point total was 30. The inspector reviewed 3 children?s records and 2 caregiver records.
This inspection included a document review, a tour of the facility, interviews, and observations.
Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.
Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five business days from today, which will be the close of business on October 7 2025. A POC submitted after this date will not appear on the public website.
Violations
5The provider did not ensure that Staff # 2 record contained the name, address, and telephone of a person to be notified in an emergency. The provider is 3 months out of compliance.
The provider did not ensure that cribs being used by infants had a tight fitting bottom cover. 4 of 8 cribs in the provider's home need tight fitting bottom covers.
Child #3 record does not contain written authorization for the provider to seek emergency medical care for the child should the parent not be located immediately. The provider is 2 days out of compliance.
The provider did not ensure that documentation of policies for the administration of medications was received by the parents of Child #3. The provider is 2 days out of compliance.
The provider did not practice the emergency evacuation drills monthly. The provider conducted a shelter in place drill in June 2025 in place of the required monthly evacuation drill.
