Inspector Notes
An unannounced, on-site monitoring inspection was initiated and completed on February 19, 2025, as a part of the licensure period. The on-site inspection began at 10:45amand ended at 11:45am. The inspector reviewed compliance in the areas listed above. There were five children present and two caregivers; the point total was 17. The inspector reviewed five children?s records and two caregiver/household member(s) records on-site.
This inspection included document review, tour of the facility, interviews, and observations.
Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.
Standard 22.1-289.035-B-1
The provider is required to obtain a completed sworn statement prior to the employee's first day of employment.
Caregiver #2, who has been employed for two weeks, did not complete a sworn statement prior to their first day of employment.
Plan of Correction: A sworn statement will be completed today for Caregiver #2. Sworn statements will be completed prior to the caregiver's first day of employment.
Standard 22.1-289.035-B-2
Providers must obtain a completed national criminal background check prior to the employee's first day of employment.
Caregiver #2, who had been employed for two weeks, did not have a completed national criminal background check.
Plan of Correction: Caregiver #2 will make an appointment with the fingerprinting agency today. Caregiver #2 was never left alone with children, but in the future, fingerprint results will be completed with results back before the caregiver begins employment.

The provider is required to obtain a central registry request prior to employment.
Caregiver #2, who has been employed for two weeks, did not have documentation of a central registry request completed prior to employment.
Plan of Correction: The provider will complete a request for a central registry for caregiver #2. The provider forgot about the law requiring a central registry request being conducted prior to employment and not having results in 30 days. Moving forward, the provider will ensure to obtain a request prior to hire.
Standard 8VAC20-800-120-A
The provider shall maintain a record for each caregiver.
The provider did not maintain a record for Caregiver #2.
Plan of Correction: The provider will ensure to create a record for caregiver #2.
Standard 8VAC20-800-170-A
The provider shall obtain from each caregiver at the time of hire a current Report of Tuberculosis Screening.
Caregiver #2 did not have documentation of a Tuberculosis Screening at the time of the inspection.
Plan of Correction: Caregiver #2 will make an appointment today to have a tb test completed.

Documentation of the orientation shall be signed and dated by the provider and assistant.
Caregiver #2 did not have documentation of orientation.
Plan of Correction: Caregiver #2 will complete and sign orientation.
Standard 8VAC20-800-680-A
Caregivers shall wash their hands with liquid soap and warm running water after diapering a child.
Caregiver #2 did not wash their hands with liquid soap and warm running water after diapering a child. Caregiver #2 applied hand sanitizer to their hands after diapering.
Plan of Correction: Caregiver #2 will be retrained on handwashing procedures for caregivers and children.

Caregivers shall ensure that children's hands are washed with liquid soap and warm running water after diapering.
Child #5's hands were not washed after diapering.

Standard 8VAC20-800-700-A
Prescription and nonprescription medications shall be given to a child only with written authorization from the parent.
Documentation of written authorization from the parent for a prescription medication was not available at the time of the inspection for Child #1.
Plan of Correction: Written authorization from the parent of Child #1 will be obtained today for the prescription medication. Moving forward, all prescription and non prescription medication will have written authorization from parents.
