Inspector Notes

An unannounced, on-site monitoring inspection was initiated and completed on January 23, 2026, as a part of the licensure period. The on-site inspection began at 11:00am and ended at 12:00pm. The inspector reviewed compliance in the areas listed above. There were three children present and two caregivers; the point total was 7. The inspector reviewed four children?s records and three caregiver/household member(s) records on-site.
This inspection included document review , tour of the facility, interviews, and observations.

A subsidy health and safety inspection (SHSI) was conducted in conjunction with the licensing inspection. Subsidy standards that are not covered in licensing standards were reviewed during this SHSI.

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Standard 8VAC20-800-180-A
The provider shall obtain for each caregiver and adult household member a current Report of Tuberculosis Screening form every two years from the date of the first screening.

Caregiver #1's record did not obtain documentation of a subsequent Report of Tuberculosis(TB) Screening form. Documentation of a previous TB screening report was expired by nine days.

Caregiver #2's record did not contain documentation of a subsequent Report of Tuberculosis (TB) Screening form. Documentation of a previous TB screening report was expired by 10 days.

Household Member #1's record did not contain documentation of a subsequent Report of Tuberculosis (TB) Screening form. Documentation of a previous TB screening was expired by four days.
Plan of Correction: The caregivers and household members will get an updated TB screening completed.
Standard 8VAC20-800-780-B
Annually, the provider shall review with the parent the emergency contact information to ensure the information is correct and obtain a parent's signed acknowledgment of the review.

The record for Child #4, enrolled for 15 months, did not contain a parent's signed acknowledgment of the review of the emergency contact information.
Plan of Correction: The parent will review the emergency contact and document the record.
Standard 8VAC20-800-800-B
The provider shall review and document in writing the each review of the emergency plan at least annually and update the plan as needed.

The provider confirmed that the emergency plan was not reviewed and documented in writing annually. Documentation of the last review in writing was twenty months ago.
Plan of Correction: The emergency plan will be reviewed and signed to indicate the annual review. Moving forward, the emergency plan will be reviewed and signed off on annually.
Standard 8VAC20-800-90-C
The family day home shall obtain documentation of additional immunizations for each child who is not exempt from the immunization requirements once every six months for children under the age of two years.

Child #4, enrolled for 15 months and 17 months old, did not have additional immunizations once every six months. Documentation of the last immunization record was nine months ago.
Plan of Correction: The provider will get a copy of the updated immunization records from the parent.
