During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

Inspector Notes

An unannounced, on-site focused inspection was initiated and completed on 3/27/2025, as a part of the licensure period. The on-site inspection began at 9:20am and ended at 10:30am. The inspector reviewed compliance in the areas listed above. There were 5 children present including the child household members and 2 caregivers; the point total was 6. The inspector reviewed 2 children?s records and 3 caregiver/household members records on-site.
This inspection included: document review, tour of the facility, interviews, and observations.
Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Standard 8VAC20-800-200-A
REPEAT VIOLATION
The provider shall orient assistants by the end of their first week of assuming job responsibilities.
Caregiver #1 and #2 have not completed orientation by the provider. Caregivers #1 and #2 assumed responsibilities over six months ago.
Plan of Correction: Provider will complete and document orientation with assistants today.
Standard 8VAC20-800-60-B
REPEAT VIOLATION
Children's records are required to contain name addresses and phone numbers of two
designated persons to contact in case of an emergency if the parent cannot be reached.

Child #1 and Child #2's did not have information on a second person to contact in case of an emergency if the parent cannot be reached. The children were present during the inspection and have been in care for at least 5 months.
Plan of Correction: Provider will request information on a second emergency contact person again today.
