Inspector Notes

An unannounced, on-site renewal inspection was conducted on 7/23/24 from 9am to 9:30am.The inspector reviewed compliance in the areas listed above. There were 2 children present and 1 caregiver; the point total was 4. The inspector reviewed 3 children?s records and 1 staff record. This inspection included:
? document review (attendance, emergency drill logs, authorization forms),
? tour of the family day home,
? observation of morning snack, arrival to family day home and free play.

Based on records review, the provider did not obtain documentation of a physical exam for a child in care within 30 days of enrollment.
Evidence: Child # 1's first day of enrollment was 2/12/24. Child # 1's physical exam has not been obtained to this date.
Plan of Correction: A copy of the exam will be requested from the parents.
