Inspector Notes
An unannounced, on-site initial subsidy health and safety inspection was initiated on 07/19/2024 and completed on 07/19/2024. The on-site inspection began at 10:00 am and ended at 11:00 am. The inspector reviewed compliance in the areas listed above. There were 2 children present, 1 caregiver, and 1 household member. The inspector reviewed 3 children?s records, 1 caregiver record and 1 household member record on-site. This inspection included document review, tour of the home, interviews, and observations.
Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.
Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the deficient practice will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on 08/02/2024. A POC submitted after this date will not appear on the public website.

The record for child #2 indicated that the child has a food allergy. Interview revealed that the vendor did not have a current written list of all children's allergies.

The record for child #2 revealed that the child has a food allergy. The record did not contain a written care plan to include instructions from a physician regarding the food to which the child is allergic and the steps to be taken in the event of a suspected or confirmed allergic reaction.
Evidence:
The water temperature in the bathroom was 150 degrees F.
Evidence:
The diaper table had a folded cotton blanket on top. There was not a nonabsorbent pad on the diaper table. The vendor acknowledged that the diaper table did not have a nonabsorbent pad that can be cleaned and sanitized after each use.

The household medication was in an unlocked cabinet in the kitchen.
Evidence:
The vendor did not have a working, battery operated radio.
