Inspector Notes

An unannounced monitoring inspection was initiated and completed on 11/18/2024 in response to a self-report received by the licensing office on 11/08/2024 relating to supervision. The inspector reviewed 2 staff records on-site. This inspection included document review, tour of the facility and interviews.

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on 11/27/2024. A POC submitted after this date will not appear on the public website.

Children under 10 years of age shall always be within actual sight and sound supervision of staff.

On 11/08/2024, 19 month old Child #1 was not within actual sight and sound supervision of staff for approximately 5 minutes after being left alone on the playground.
Plan of Correction: All children will remain within actual sight and
sound supervision of staff at all times.

The operational responsibilities of the licensee includes ensuring that the center's activities, services, and facilities are maintained in compliance with these standards, the center's own policies and procedures that are required by these standards, and the terms of the current license issued by the department.

The center's policy on supervision states: ?teacher's complete Name to Face Transitions at the first and last threshold when moving children out of a designated space.?
On 11/08/2024, Staff #1 and Staff #2 did not conduct name-to-face counts using the Child Supervision Record as required by policy after leaving the playground and reaching the last threshold, resulting in 19 month old Child #1 being left alone and unsupervised on the playground for approximately 5 minutes.
Plan of Correction: Staff will complete Name-to-Face Transitions Check at the first and last threshold every time they move children out of a designated space.

Staff was retrained on how to properly complete Name-to-Face Check and how to log it on our supervision tool on 11/14/2024.

Leadership team will continue to support staff during transitions and ensure Name-to-Face checks are being completed during transitions.
