Inspector Notes

An unannounced, on-site renewal inspection was initiated on 08/21/2024 and completed on 08/21/2024, as part of the conditional licensure period. The on-site inspection began at 3:10 pm and ended at 5:00 pm. The inspector reviewed compliance in the areas listed above. There were 19 children present and 7 staff. The inspector reviewed 6 children's records and 4 staff records on-site.

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documentation on the violation notice issued to the program.

Based on record review and interview, the center failed to obtain a copy of the results of a criminal history record information check and a search of the child abuse and neglect registry or equivalent registry from any state in which the individual has resided in the preceding five years.

Evidence:
1. A criminal record check and central registry check is not on file for a state outside of Virginia that staff 1 has resided in within the past five years.
2. Administrative staff confirmed that these out-of-state background checks were not on file for staff 1.
Plan of Correction: Administrative staff stated that these out-of-state background checks will be requested immediately. All Virginia background checks are on file.
