Inspector Notes
An unannounced inspection was initiated and completed on 9/18/2025 in response to a complaint received by the licensing office on 9/16/2025 relating to allegations of staffing and supervision. This inspection included document review, tour of the facility, interviews, and observations.
The preponderance of evidence gathered during the investigation supports the allegation; therefore, the complaint is determined to be valid. Information gathered during the inspection determined non-compliance with applicable standards or law, and the violations are documented on the violation notice issued to the program.
Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five business days from today, which will be the close of business on 9/26/2025. A POC submitted after this date will not appear on the public website.
Violations
3The Child Supervision Record for the infant classroom documented that the classroom was over ratio numerous times on the following dates: 9/11/2025, 9/12/2025, 9/15/2025, and 9/16/2025 .
The Child Supervision Record for the Preschool 3 classroom documented that the classroom was over ratio on the following dates: 9/03/2025 from approximately 2:30pm-3:00pm and 9/10/2025 from approximately 2:30pm-3:12pm.
The Child Supervision Record for the school age classroom documented that the classroom was over ratio on the following dates: 9/02/2025, 9/03/2025 and 9/05/2025.
A staff member stated on 9/16/2025 the school age classroom (school age eligible children up to 12 years old) was out of ratio. The staff member stated they could not find the child supervision record for 9/16/2025, but had 26 enrolled in the program and had only one staff member working the classroom. Staff stated she thought the ratio was 1 staff per twenty children. The staff member stated this has occurred from approximately from 8/25/2025 through 9/17/2025. . .
Another staff member stated that in our afterschool program, "I am frequently left alone with 20 children ranging in age from 5 to 11 years old. This has been occurring almost daily for the past four months."
