Inspector Notes

An unannounced, on-site monitoring inspection was completed on 04/29/2025, as a part of the licensure period. The on-site inspection began at 11:00 am and ended at 12:00 pm. The inspector reviewed compliance in the areas listed above. There were 4 children present and 1 staff. The inspector reviewed 4 children?s records and 1 staff record. This inspection included a document review, tour of facility, interviews, observations and measurements. Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Standard 8VAC20-800-100-A
The provider shall obtain documentation of a physical examination prior to a child's attendance or within 30 days after the first day of attendance.

There were no physical exam records available for Child #2 and #3, who had been in care for over seven months.
Plan of Correction: Provider requested from parent.
Standard 8VAC20-800-140-B-3
Provider shall have current certification in cardiopulmonary resuscitation (CPR), as appropriate to the age of the children in care, from the American Red Cross, American Heart Association, American Safety and Health Institute, or the National Safety Council, or current CPR certification issued within the past two years by a community college, a hospital, a rescue squad, or a fire department.

Provider's CPR certification expired a month ago.
