Inspector Notes
A self-report investigation was initiated on July 22,2024 and completed on September 16, 2024, in response to a self-report received by the licensing office on July 19, 2024, relating to supervision. The inspector reviewed one children?s record and two staff records electronically on August 8, 2024. This inspection included document review, observations, and interviews. Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.
Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the deficient practice will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on September 23, 2024. A POC submitted after this date will not appear on the public website.
Violations
4Evidence:
1.The record for Staff #3 contained an expired criminal history check and an expired central registry search, which were completed over five years ago.
1.Staff #1 confirmed that Child #1 (two years old) was left alone for approximately 15-20 minutes.
a. Both Staff #1 and Staff #2 stated that it was evident that Child #1 had been crying during the time of the incident.
b. Staff #3 stated that Child #1 was crying at the time they retrieved the child from the classroom.
2.Staff #2, one of two teachers assigned to Room 3 at the time of the incident, stated that she was not aware that Child #1 had been left alone and unsupervised in the classroom until the child was returned to the group still out on the playground, by Staff #3.
Evidence:
1.On July 19, 2024, Child #1 (two years old) was left unsupervised in Room 3. The child was found alone in the classroom, by the center?s custodian approximately 15-20 minutes after the staff had taken the rest of the class outside.
2.Staff #1 (program director) confirmed that Child #1 was left alone in Room 3, without staff supervision.
Evidence:
On July 19, 2024, the center did not follow their policies related to child supervision.
1. The classroom supervision policy states that ?staff must never, ever, for any reason, leave a child unattended.?
a. Child #1 (2 years old) was left in Room 3, alone and unsupervised for approximately 15-20 minutes.
2.The playground supervision policy states that ?teachers should know how many children are in their care at all times? and lists times in which teachers should count children, to include before taking children outside and when children are outside.
a. Staff #3 confirmed that they did not count children in the group while outside.
3.The reporting suspected child abuse and neglect policy states that once the director is notified, ?the director will immediately make a report to the Department of Social Services.? The center defines neglect as ?the failure to provide for a consumer?s basic needs or the failure to protect a consumer from harm.?
a. Staff #1 (program director) confirmed that after the incident, a report was not made regarding Child #1 (two years old) being left alone in the classroom without staff supervision for approximately 15-20 minutes.
