8VAC20-780 Physical Plant.

During the inspection, the inspector reviewed the areas listed above. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

Inspector Notes

An unannounced monitoring inspection was initiated in response to a self-report received by the licensing office on 8/18/25 relating to a serious injury. The Department issued a Special Order to mandate training for all staff and contact parents, guardians, or other responsible persons regarding health and safety violations on 1/3/25 as a result of high-risk violations. The inspector reviewed zero children?s records and zero staff records.This inspection included document review and interviews. Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program. Though not a part of the original self report, violations that were not reported were found related to administration and are documented on the violation notice issued to the program.

Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on 11/17/25. A POC submitted after this date will not appear on the public website.

Repeat Violation
'Where playground equipment is provided, resilient surfacing shall comply with minimum safety standards (six inches).
On 8/18/25, when a serious injury of a child occurred, there was not the minimum required amount of resilient surfacing on one of the center's outdoor play areas.

Repeat Violation
The center shall maintain a written record of children's serious and minor injuries in which entries are made the day of occurrence. The record shall include the following: staff and parent signatures or two staff signatures.
An injury report for a serious injury on 8/18//25 did not contain staff and parent signatures or two signatures, which is required.
