Repeat Violation
The center must request repeat background checks every five years.
A repeat criminal history background check and central registry search for Staff #5 were not obtained until two months after the background checks had expired.
Plan of Correction: Center Director will check files quarterly for upcoming renewals.
Standard 8VAC20-780-160-A
Each staff member and individual from an independent contractor shall submit documentation of a negative tuberculosis (TB) screening. Documentation of the screening shall be submitted at the time of employment and prior to coming into contact with children and shall have been completed within the last 30 calendar days of the date of employment.
TB documentation for Staff #1 and Staff #3 was not submitted at the time of employment. TB documentation for Staff #2 was not completed within 30 calendar days of employment.
Plan of Correction: Center Director will require TB screenings be submitted by new hires before scheduling orientation.
Standard 8VAC20-780-160-C
At least every two years from the date of the first initial screening or testing, staff members shall obtain and submit the results of a follow-up tuberculosis (TB) screening.
The TB documentation for Staff #4 was expired on the day of the inspection. The results had been expired for three months.

The Virginia Department of Education-sponsored orientation course shall be completed within 90 calendar days of employment.
There was no documentation in the record of Staff #3 (employed for four months) to show that The Virginia Department of Education-sponsored orientation course was completed.
Plan of Correction: Center Director will schedule all new hires that have not completed the VDOE pre-service training with on-site course completion during the onboarding process.

Repeat Violation
Where playground equipment is provided, resilient surfacing shall comply with minimum safety standards (six inches).
Three inches of resilient surfacing was measured under the monkey bars on the main playground. Children were observed playing on the playground on the day of the inspection.
Plan of Correction: Mulch has been redistributed to fall zone areas. Administration will monitor fall zones for resilient surfacing levels and build up or replace, as needed.

Documentation shall be maintained of shelter-in-place drills that includes all the required information.
Documentation for a shelter-in-place drill completed in 2025 did not contain all the required information.
Plan of Correction: Administration will review drill log quarterly to ensure drill documentation has all required information.

Staff records shall be kept for each staff person and contain all the required information.
There was no documentation in the record of Staff #2 to show that the individual possesses orientation, as required.
