Inspector Notes

An announced initial inspection was conducted on April 15, 2024, to determine compliance with licensing standards. The inspector was on site from 12:00 pm until 1:30 pm. The owner and program director were present during the inspection. The facility plans to provide care to children ages 12 weeks through five years. Measurements of the facility were taken to determine capacity. The calculations indicate a capacity of 25 children. Classrooms are equipped with a variety of age-appropriate equipment and supplies including materials for gross motor, art, literacy, and dramatic play. The facility will provide meals. Standards related to supervision and programing could not be determined as the facility is not yet fully operational. Policies and procedures are in place. Coverage of CPR, first aid, and daily health training were verified. A total of three staff records were reviewed.
A conditional (6 month) license will be recommended. An unannounced follow-up inspection will be conducted within 60 days to determine compliance with standards not determined at the initial inspection.

Based on review of three staff records and interview, the licensee did not obtain documentation of a negative tuberculosis screening at the time of employment and prior to coming into contact with children. The documentation shall have been completed within the last 30 calendar days of the date of employment.

The record of staff #2, employed 04/15/2024, contained documentation of a negative tuberculosis screening dated 02/05/2024, exceeding 30 days prior to employment.
