An unannounced, on-site monitoring inspection was initiated on June 3, 2025, and completed on June 3, 2025, as a part of a request to modify the license to serve children ages three months through seven years, 11 months. The on-site inspection began at 10:00 am and ended at 12:45 pm. The inspector reviewed compliance in the areas listed above.
