Standard 22.1-289.035-B-4
The center is required to obtain background checks from any state in which the individual
has resided in the preceding five years. S4, employed for 14 days, did not have
documentation of requesting a central registry check. S5, employed for over 2 months, did not have documentation of requesting a central registry check.
Plan of Correction: Both individuals had Virginia State background checks completed as well as 2 out of 3 out of state background checks. 3rd background check required staff member to create an account with the state (NC) and to request background check. Both staff members have completed the account and are awaiting the request.
Standard 8VAC20-780-240-B
Staff shall complete orientation training prior to the staff member working alone with children and no later than 7 days of the date of assuming job responsibilities. The file for S10 employed for 15 days did not contain documentation of orientation training.
Plan of Correction: Staff member was not left alone with children during 15 days of employment. Staff member was terminated before Center received all documents back from staff member.
Standard 8VAC20-780-240-E
Staff shall complete within 30 days of the first day of employment an orientation training in first aid and cardiopulmonary resuscitation. The record for S3 and S5 did not document this orientation.
Plan of Correction: Staff will be required to sign line indicating CPR/First Aid orientation is completed as well as the line indicating full orientation is completed as well (both are on same form already)
Standard 8VAC20-780-245-A
Staff shall complete a minimum of 16 hours of training annually, appropriate to the age of children in care. The record for S1 documented 12 of the 16 hours of required training
for 04/2024 - 04/2025.
Plan of Correction: Please see attached form with training log of competed hours. Staff member received 17 hours of training.
Standard 8VAC20-780-270-A
The center shall ensure that areas and equipment of the center, inside and outside,
are maintained in a clean, safe and operable condition. The chain link gate entering the Pre-K playground had sharp and exposed wire at the bottom of the gate.
Plan of Correction: Center was aware and had already requested repair of gate. Center was waiting on maintenance to schedule appointment to complete.
Standard 8VAC20-780-40-J
The center is required to update annually the injury prevention procedures based on
documentation of injuries and a review of the activities and services. No documentation was available to show compliance with this requirement.
Plan of Correction: While injury reports are kept on file and logged into a computer database, an official form to show compliance of annual review has been created.
Standard 8VAC20-780-550-P
Repeat Violation
The center shall maintain written records of children?s injuries that contain all of the
required information. Injury reports dated 09/29/2025 and 10/28/2025 did not document future actions to prevent recurrence of the injury. Injury reports dated 10/22/0225 and 10/28/2025 did not document the date and time of parental notification.
Plan of Correction: Staff will receive additional training on process to ensure completion of incident reports. Management will be reminded on making sure incidents are completed.
Standard 8VAC20-780-70
Repeat Violation
Each center shall maintain a record for each staff that contains all the required information. The record for S3 did not document a second employment reference. The record for S4 did not contain the name, address, and telephone number of a person to be notified in an emergency.
