An unannounced, on-site code compliance inspection was conducted on June 17, 2025. The on-site inspection began at 2:15 p.m. and ended at 4:00 p.m. The inspector reviewed compliance in the areas listed above. There were 21 children present with 4 staff. The inspector reviewed 5 children?s records and 5 staff records on-site. This inspection included document review, tour of the facility, interviews, and observations

Background checks must be repeated every five years in accordance with the Code of Virginia. The most recent national fingerprint check for Staff 1, Staff 2, Staff 3, Staff 4, and Staff 5 were more than 5 years old. The search of the central registry for Staff 1 was more than 5 years old.
Plan of Correction: The center director will contact OBI to get needed background checks.
