During the inspection, the inspector reviewed the areas listed above. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

Inspector Notes

An unannounced monitoring inspection was completed on 03/20/2026 as a part of the licensure period. The on-site inspection began at 11:30am and ended at 1:00pm. The inspector reviewed compliance in the areas listed above. There were 21 children present and 5 staff. The inspector reviewed 3 children?s records and 4 staff records. This inspection included: document review, tour of the facility, interviews, observations and measurements. A subsidy health and safety inspection (SHSI) was conducted in conjunction with the licensing inspection. Subsidy standards that are not covered in licensing standards were reviewed during this SHSI, and this single inspection report reflects findings related to licensing standards and subsidy standards. Information gathered during the inspection determined noncompliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on April 1st. A POC submitted after this date will not appear on the public website.

Standard 8VAC20-780-270-A
Areas and equipment of the center, inside and outside, shall be maintained in a clean and safe condition. On the playground there was a water table that was not covered. There was brown water trash and a box of cigars in it.
Plan of Correction: The water table was immediately emptied, cleaned, sanitized, and removed from use until it could be properly maintained. All debris, trash, and prohibited items (including the cigar box) were removed from the playground. The entire playground area was inspected to ensure it was clean, safe, and free of hazards. Playground will be properly checked every day at opening.
Standard 8VAC20-780-280-G
If hazardous substances are not kept in original containers, the substitute
containers shall clearly indicate their contents. There was a spray bottle containing a blue liquid that was not labeled to indicate the contents of the substitute container.
Plan of Correction: All hazardous substances transferred Into secondary containers will be clearly labeled with the name of the contents. Only properly labeled containers will be permitted for use within the facility. Unlabeled containers will be immediately discarded or removed from use.
Standard 8VAC20-780-330-B
Where playground equipment is provided, resilient surfacing shall comply with
minimum safety standards when tested in accordance with the procedures
described in the American Society for Testing and Materials standard F1292-99. There was a toy on the playground that spins and did not have resilient surfacing underneath it.
Plan of Correction: The spinning equipment was Immediately taken out of use to prevent access by children.
Standard 8VAC20-780-340-F
Children under 10 years of age always shall be within actual sight and sound supervision of staff.

Five children under age 10 years were not in sight and sound supervision of a staff during nap. The door to the classroom was closed and staff #1 was asleep at a table with her head down. The LI opened the door and walked around the classroom and the staff member did not move. About 2 children were awake with the television on. Center staff came into the classroom to wake her up.

In a second classroom 5 additional children under the age of 10 years were not in sight and sound supervision of a staff during nap. The door to the classroom was closed and staff #2 was laying on the floor with no shoes and a blanket covering her entire body. The Director came into the classroom to wake her up
Plan of Correction: Both staff members involved were immediately terminated due to failure to ensure the safety and supervision of children. We hired qualified replacement staff who meet all state licensing requirements. Newly hired staff completed mandatory training on active supervision. Staff will continue be trained on the seriousness and non-negotiable nature of supervision at all times. Training will emphasize that sleeping while supervising children is strictly prohibited and will result in immediate termination. Staff were required to sign documentation acknowledging understanding of these policies.
Standard 8VAC20-780-370-3
Infants are required to be provided daily outdoor time. The Director stated the children do not play outside daily and had not been outside for several days.
Plan of Correction: A daily schedule is implemented to ensure infants receive outdoor time every day, weather permitting. Outdoor time may include use of strollers, blankets, or designated infant-safe play areas. In cases of unsafe weather conditions, infants will receive alternative indoor sensory and gross motor activities.
Standard 8VAC20-780-380-A-1
Toddlers and preschoolers are required to be provided daily outdoor time. The Director stated the children do not play outside daily.
Plan of Correction: Effective immediately, all toddlers and preschoolers are being scheduled for daily outdoor play time. Class schedules have been adjusted to ensure outdoor time is included every day, weather permitting. Staff were informed of the requirements and expectations for daily outdoor activities.
Standard 8VAC20-780-450-A
Cots and mats used by children shall have linens consisting of a top cover and a bottom cover. Four children were observed on cots that did not consist of both a bottom and top cover.
Plan of Correction: Each child assigned a cot or mat will be provided with: A bottom cover (sheet or fitted sheet) , A top cover (blanket or appropriate covering). Extra linens will be maintained on- site to ensure availability at all times. Linens will be laundered regularly in accordance with sanitation guidelines.
Standard 8VAC20-780-500-B
When a child's clothing or diaper becomes wet or soiled, the child shall be cleaned and changed immediately. There was a 4 year-old child #4 with a wet diaper. Staff A did not change the diaper immediately because there was not enough staff to watch the children in her room to take him to the changing area.
Plan of Correction: Classroom supervision was adjusted to ensure all children were properly supervised while meeting individual care needs. Staff A was counseled on the requirement that children must be changed immediately, regardless of staffing challenges.
Standard 8VAC20-780-570-I
A one-day's emergency supply of disposable bottles, nipples, and commercial formulas appropriate for the children in care shall be maintained at the center. The center did not have a one-day's supply of emergency formula or bottles. Staff stated they did not know it was required.
Plan of Correction: The center immediately obtained and stocked a one-day emergency supply of:
disposable bottles, nipples and commercial formula appropriate for infants in care. Supplies are now stored in a designated, easily accessible location for emergency use.
Standard 8VAC20-780-80-A
For each group of children, the center shall maintain a written record of daily
attendance that documents the arrival and departure of each child in care as it
occurs. There were 21 children in care and the written record of attendance had 12 of the 21 children checked in with arrival time.
Plan of Correction: Attendance sheets will be completed and updated daily in real time to reflect the accurate number of children in the building at all times. Each child's arrival and departure time will be recorded immediately as it occurs. Attendance records wiU be maintained in each classroom and readily accessible for review.
