During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

Inspector Notes

An unannounced, on-site monitoring inspection was initiated and completed on January 22, 2025. The on-site inspection began at 10:30 a.m. and ended at 11:25 a.m. The inspector reviewed compliance in the areas listed above. There were 28 children present and 4 staff. The inspector reviewed 5 children?s records on-site. This inspection included document review, tour of the facility, and observations. Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on January 30, 2025. A POC submitted after this date will not appear on the public website.

Hazardous substances shall be kept in a locked place that prevents access by children.

A plug-in air freshener was accessible to children in the Pre-Primary classroom.
Plan of Correction: Removed the air freshener from the downstairs hallway

If hazardous substances are not kept in original containers, the substitute containers shall clearly indicate their contents.

A substitute container containing a cleaning solution did not indicate its contents.

Children's hands shall be washed with soap and running water after toileting.

Child #5's hands were not washed after the child's diaper was changed.

Standard 8VAC20-780-500-B
A nonabsorbent surface for diapering shall be used. For children younger than three years, this surface shall be a changing table or countertop designated for changing. Disposable diapers shall be disposed in a storage system that is either foot-operated or used in such a way that neither the staff member's hand nor the soiled diaper touches an exterior surface of the storage system during disposal. The diapering surface shall be cleaned with soap and water and sanitized after each use.

Staff #1 was observed changing Child #5's (age: 2 years) diaper on an absorbent mat on the floor. The diapering surface was not cleaned with soap and water and sanitized after use. Diapers were disposed in a storage system that was not foot operated or used in such a way that neither the staff member's hand nor the soiled diaper touches an exterior surface of the storage system during disposal.
Plan of Correction: I spoke to the teachers about using changing table, washing hands after diaper change, cleaning the changing table with soap and water. It is very important to use the foot operated trash can.
