During the inspection, the inspector reviewed the areas listed above, to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

Inspector Notes

An unannounced, on-site monitoring inspection was initiated and completed on August 7, 2025. The on-site inspection began at 9:00am and ended at 11:00am. The inspector reviewed compliance in the areas listed above. There were 11 children present and 3 staff. The inspector reviewed 5 children?s records and 4 staff records on-site. This inspection included document review, tour of the facility, interviews, and observations. Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Standard 8VAC20-800-210-A
Caregivers shall obtain a minimum of sixteen clock hours of training annually in
areas relevant to their job responsibilities. Two of the four staff records reviewed did not have the minimum of 16 hours of annual training.
Plan of Correction: The provider responded with the following: The staff members will complete 16 hours of annual training and a year training log will be completed.
Standard 8VAC20-800-520-A
Cribs shall be provided for children from birth through 12 months of age and
for children over 12 months of age who are not developmentally ready to sleep on
a cot, rest mat, or bed. Two children ages 13 months and 15 months are sleeping in cribs and have no documentation regarding not developmentally ready to sleep in a cot, rest mat, or bed.
Plan of Correction: The provider responded with the following: The provider will speak with the parents should they want their child to stay in the crib longer. Otherwise, the staff will transition the children at 12 months to a cot.
Standard 8VAC20-800-680-B
Caregivers shall ensure that children's hands are washed with liquid soap and
warm running water before eating. A child did not wash their hands before breakfast
Plan of Correction: The provider responded with the following: The staff will ensure that the children?s hands are being washed before eating.
Standard 8VAC20-800-700-B
The parent's written authorization for medication shall expire or be renewed
after 10 working days. Child #1 has a prescriptive medication that does not have a current written authorization within the past 10 days.
Plan of Correction: The provider responded with the following: The medication for Child #1 was not being used at the center and will be returned to the parent. Should a child have a medication, written authorization will be renewed every 10 days.
