During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation. .

Inspector Notes

An unannounced, on-site monitoring inspection was initiated and completed on 5/20/2025, as a part of the licensure period. The Department issued a Special Order on 5/12/2025 to place the licensee on probation for a period of three months as a result of high-risk violations. The on-site inspection began at 12:10pm and ended at 3pm. The inspector reviewed compliance in the areas listed above. There were 78 children present and 21 staff. The inspector reviewed 8 children?s records and 6 staff records on-site. This inspection included document review, tour of the facility, interviews, and observations. Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

The center is required to obtain background checks from any state in which the individual has resided in the preceding five years. The sex offender registry check in the record Staff #2 was obtained more than two months after Staff #2's first date of employment. The sex offender registry check in the record Staff #3 was obtained one day after Staff #3's first date of employment.
Plan of Correction: The center completed the checks as soon as they became aware they were missing. The center will ensure background checks are completed within required time frames.
Standard 8VAC20-780-70
Centers are required to maintain a record for each staff person which includes documentation that two or more references as to character and reputation as well as competency were checked before employment or volunteering. The record for Staff #3, who has been employed for more than one month, did not contain documentation that 1 of 2 required references were checked. The record for Staff #5, who has been employed for four months, did not contain documentation that 1 of 2 required references were checked.
