Inspector Notes
An announced initial monitoring inspection was conducted 3/26/24 from approximately 9:30am-11:25am. There were 2 children (7 points) directly supervised by 2 caregivers. The home, 2 caregiver records, 2 children?s records, 3 household member?s records, evacuation drills, emergency supplies, and policies were inspected. There were no medications on site today. Children were observed playing freely and interacting with caregiver. There was an assortment of age appropriate materials for children. Areas of non-compliance are identified in the Violation Notice.

1. HHM#1 did not have completed sworn statement or affirmation on file.
2. The provider stated they thought sworn statement was only needed for caregivers.
Evidence:
1. No references in caregiver/assistant's file.
2. Provider stated they were unaware reference was needed.
Evidence:
1. The provider's home did not have a landline telephone.
2. The provider stated they were not aware that radio needed to have weather band.
Evidence:
1.The only first aid kit in the home, only contained 1 triangular bandage.
2. The provider stated they were unaware that more than 1 triangular bandage was required.
Evidence: The radio in the home was AM/FM and not weather band.
Evidence: The home's menu did not include a second vegetable or a fruit at lunch.
