Inspector Notes
An unannounced monitoring inspection was conducted 6/7/2024 from 9:20-10:40am. There were 5 children (15 points) directly supervised by 2 caregivers. The home, 2 caregiver and 1 household member records, 3 children?s records, evacuation drills, emergency supplies, and policies
were inspected. There were no medications on site today. There was an assortment of age appropriate materials for children. Areas of non-compliance are identified in the Violation Notice.

5Evidence: Caregiver #1's record did not contain documentation of the education and training as required by 8VAC20-800-230
Evidence: The telephone was connected to a land line but required electricity to operate.
Evidence: On 6/7/2024,last entry of children in attendance was 6/5/2024.
Evidence:
1. Child #1's record did not contain name, office address and telephone number of the child's physician.
2. Child #1's record did not contain proof of the child's age and identity and the names and addresses of previously attended child day care and schools as required by 8VAC20-800-
80;
3.Child #1's record did not contain immunization records for the child as required by 8VAC20-800-90;
4. Child #1's record did not contain results of the health examination for the child as required by 8VAC20-800-100
5. Child #1's record only had name, address and telephone number of one designated persons to contact in case of an emergency if the parent cannot be reached.
Evidence: No documentation of emergency evacuation procedure practice 5/2024 (Date Of Last drill 4/25/2024).
