Inspector Notes

An unannounced, on-site renewal inspection was conducted on 9/20/2024, as a part of the conditional licensure period. The on-site inspection began at 10:30am and ended at 11:40am. The inspector reviewed compliance in the areas listed above. There were 5 children present and 3 caregivers; the point total was 15. The inspector reviewed 3 children?s records and 1 staff record on-site. This inspection included document review of. Injury logs, policies and procedures, emergency drill logs, authorization forms, tour of the home, interviews, and observations.
Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Standard 8VAC20-800-100-A
The provider shall obtain documentation of a physical examination prior to a child's attendance or within 30 days after the first day of attendance.

There was no physical exam records available for child #1, who had been in care for over five months.
Plan of Correction: Provider to request physical exam from parent today. Provider will ensure physical exam documentation is obtained prior to or within 30 days of a child's attendance.
Standard 8VAC20-800-330-A
REPEAT VIOLATION
A landline telephone, excluding a cordless or cell phone, shall be available, operable, and accessible during the family day home's hours of operation. An operable landline telephone is one that does not require electricity to operate. Cordless or cell phones may be used in addition to the landline telephone.

There was not a telephone available, operable and accessible during the family day home's hours of operation that did not require electricity to operate.
Plan of Correction: Provider ordered landline phone that does not require electricity.

Before attendance, documentation of immunizations is to be obtained for each child.

There was no documentation of immunizations for child #1, who was in care during the inspection and had been in care for five months.
Plan of Correction: Provider to request documentation from parent at pick up today. Provider will ensure immunization record prior to a child's first day.

Documentation of additional immunizations is required once every six months for children under the age of two years. '

There was no documentation of immunizations for child #1,19 months old and has been in care over 6 months.
Child #2, 23 months old, last documented immunization was over 11 months ago.
Plan of Correction: Provider to request updated immunization records form parents today. Provider will review records to ensure updated immunizations are obtained every six months for children under 2 years old and one between child's fourth and sixth birthdays.
