Inspector Notes
An unannounced, on-site renewal inspection was conducted on 9/20/2024, as a part of the conditional licensure period. The on-site inspection began at 10:30am and ended at 11:40am. The inspector reviewed compliance in the areas listed above. There were 5 children present and 3 caregivers; the point total was 15. The inspector reviewed 3 children?s records and 1 staff record on-site. This inspection included document review of. Injury logs, policies and procedures, emergency drill logs, authorization forms, tour of the home, interviews, and observations.
Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

4There was no physical exam records available for child #1, who had been in care for over five months.
A landline telephone, excluding a cordless or cell phone, shall be available, operable, and accessible during the family day home's hours of operation. An operable landline telephone is one that does not require electricity to operate. Cordless or cell phones may be used in addition to the landline telephone.
