During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

Inspector Notes

An unannounced, on-site monitoring inspection was initiated and completed on 3/16/2026 as a part of the licensure period. The on-site inspection began at 9:45am and ended at 11:05am. The inspector reviewed compliance in the areas listed above. There were 10 children present and 4 caregivers; the point total was 29. The inspector reviewed 4 children?s records and 5 caregiver/household members records on-site.
This inspection included: document review, tour of the facility, interviews, and observations.

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on 3/23/2026. A POC submitted after this date will not appear on the public website.

Providers must obtain a completed national criminal background check prior to the employee's first day of employment.

The updated TB screenings for Caregivers #1 and #2 and Household member #1 was not completed and was overdue by a month.

REPEAT VIOLATION
Caregivers shall obtain a minimum of sixteen clock hours of training annually in areas relevant to their job responsibilities.

Caregivers #1 and #2 did not obtain required training hours.
Plan of Correction: Caregivers and provider have scheduled classes for continued education.

The provider shall ensure that each caregiver receives training regarding the emergency evacuation, emergency relocation, and shelter-in-place procedures on an annual basis.

Documentation shall be maintained of emergency evacuation and shelter-in-place drills that includes the method used for notification, the number of caregivers participating, the number of children participating and for emergency evacuation drills only, weather conditions.

Documentation of drills did not contain required information.

REPEAT VIOLATION
Documentation of additional immunizations is required once every six months for children under the age of two years. '

Child #1, turned 2 years old while in care, last documented immunization was over 30 months ago.
Child #2, turned 2 years old while in care, last documented immunization was over 11 months ago.
Child #4, 13 months old, last documented immunization was over 9 months ago.
Plan of Correction: Parents have been asked to provide their most recent vaccination documents to provider.
