Inspector Notes

An initial subsidy inspection was conducted on 2/20/24 from 9:45 am to 10:45 am. There were 2 children (5 points) in care, that included one resident child. The inspector reviewed 1 child's record and one caregiver's record. A tour of the home was conducted. The first aid supplies were reviewed.
Information gathered during the inspection determined areas of non-compliance with applicable regulations or law, and violations were documented on the violation notice issued to the vendor.

Standard 8VAC20-790-190-B
Based on record review and staff interview, one child's record was not complete.
Evidence:
1. The vendor stated that Child # 1's first day of enrollment was 12/1/24.
2. Child # 1's record was missing the following information: documentation of proof of identification, name, address and contact information for the parents, emergency contact information, information for person(s) authorized to pick up child, written authorization for emergency medical care, written statement about pick up for ill child, written parent agreement.
Plan of Correction: I will get all the missing information for the child.
Standard 8VAC20-790-390-E
Based on staff interview, the diapering surface was not non-absorbent.
Evidence: The vendor stated that she has used the couch or the floor to change diapers, and that she did not have a non absorbent pad or over to place on the surface where she changes diapers.
Plan of Correction: I will make sure that the diapering surface is non absorbent.
Standard 8VAC20-790-410-A-1
Based on observation, the first aid kit did not have all required items.
Evidence: The first aid kit was missing the thermometer and disposable gloves.
Plan of Correction: I will get the missing items.

Based on staff interview, the vendor did not have a written emergency preparedness plan that addresses caregiver responsibility and home readiness with respect to emergency evacuation, relocation, lockdown, and shelter-in-place procedures.
Evidence: The vendor stated that she did not have an emergency plan prepared.
Plan of Correction: The plan will be written to include plan for evacuation, shelter in place and lockdown.
