Inspector Notes

An unannounced, on-site renewal inspection was initiated on 9/6/2024 and completed on 9/6/2024. The on-site inspection began at 10:10 am and ended at 11:45 am. The inspector reviewed compliance in the areas listed above. There were four children present and two staff. The inspector reviewed five children?s records and four staff records on-site. This inspection included document review, tour of the facility, interviews and observations.

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Based on record review, the center failed to obtain a copy of the results of a criminal history record information check, a sex offender registry check, and a search of the child abuse and neglect registry or equivalent registry from any state in which the individual has resided in the preceding five years.

Evidence:
Staff #2 did not have a sex offender registry check and a search of the child abuse and neglect registry or equivalent registry from any state the individual has resided in the preceding five years in their staff record.
Plan of Correction: The director will have a sex offender registry check and a search of the child abuse and neglect registry or equivalent registry from any state the individual has resided in the preceding five years completed and documented in the staff's record.
