During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

Inspector Notes

An unannounced, on-site monitoring inspection was initiated on May 13, 2025, and completed on May 13, 2025, as a part of the licensure period. The on-site inspection began at 10:30 am and ended at 11:45 am. The inspector reviewed compliance in the areas listed above. There were two children present and one caregivers; the point total was four. The inspector reviewed one child?s records and one caregiver/household member(s) records on-site. This inspection included, document review, tour of the facility, interviews, and observations.
Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Providers shall maintain current certification in cardiopulmonary resuscitation (CPR), as appropriate to the age of the children in care, from the American Red Cross, American Heart Association, American Safety and Health Institute, or the National Safety Council, or current CPR certification issued within the past two years by a community college, a hospital, a rescue squad, or a fire department. The record of caregiver #1 contained documentation of CPR certification that expired 29 days ago.
Plan of Correction: Per the provider: I have a class scheduled on May 20, 2025.

Providers shall maintain current certification in first aid from the American Red Cross, American Heart Association, American Safety and Health Institute, or the National Safety Council, or current first aid certification issued within the past three years by a community college, a hospital, a rescue squad, or a fire department. The record of caregiver #1 contained documentation of first aid certification that expired 29 days ago.
Plan of Correction: Per the provider: I have a class scheduled for May 20, 2025.
Standard 8VAC20-800-210-A
In addition to satisfactory completion of first aid training and CPR training, caregivers shall obtain a minimum of sixteen clock hours of training annually in areas relevant to their job responsibilities. The record of caregiver #1 contained documentation of 13.5 hours of annual training during the period of August 2023 through August 2024.
Plan of Correction: Per the provider: I will take an additional class to complete the missing hours.

Shelter-in-place procedures shall be practiced a minimum of twice per year. For the period of August 2023 through August 2024, there were no shelter-in-place drills practiced.
