Inspector Notes

An announced initial inspection was conducted on 7/10//2024 between the hours of 9:45am-11:45am. There were two children in care supervised by the provider. Children were observed having morning snack and playing freely. The inspector verified the manufacture date for one crib. A variety of age appropriate materials were observed in the family day home. The inspector reviewed 1 caregiver record, 3 household member records, 2 children's records, and the Emergency Preparedness Plan submitted by the facility to ensure documentation was complete. The information gathered during the inspection determined violations with applicable standards or law. Areas of non-compliance are identified in the Violation Notice.

Based on record review, a sworn statement was not obtained for 1 caregiver and 2 household members.

Evidence:
1. The record for Caregiver #1 did not contain documentation of a sworn statement.
2. The records for Household Member #1 and Household Member #2 did not contain documentation of a sworn statement.
Plan of Correction: Sworn statements will be completed and evidence of completion will be provided to the department within 10 days.

Based on observation, potentially poisonous substances such as disinfectants, were not stored in areas inaccessible to children.

Evidence:
1. Two cans of disinfectant spray were observed in the bathroom accessible to children.
Plan of Correction: The disinfectant sprays were removed during inspection.
