During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. Inspector Notes

An unannounced, on-site monitoring inspection was initiated and completed on 10/15/2024, as a part of the conditional licensure period. The on-site inspection began at 9:15am and ended at 10:20am. The inspector reviewed compliance in the areas listed above. There were 2 children present and 1 caregiver; the point total was 5. The inspector reviewed 4 children?s records and 4 caregiver/household members records on-site.

This inspection included document review, tour of the facility, interviews, and observations.

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Standard 8VAC20-800-60-B
Providers are required to keep a separate record for each child enrolled that contains all of the required information. The record for Child #1 was missing the policy number of the child's medical insurance and written authorization for emergency medical care should an emergency occur and the parent cannot be located immediately. The child was present during the inspection and had been in care for three months.
Plan of Correction: The information and authorization was requested from the parent.
