Standard 8VAC20-800-100-A
The provider shall obtain documentation of a physical examination prior to a child's attendance or within 30 days after the first day of attendance. There were no physical exam records available for Child #1 and #2, who had been in care for four months.

Standard 8VAC20-800-60-B
An up to date child record that contains all required information shall be maintained by the provider. The records for Child #1, 2, and 3 did not contain the name and policy number of the child's medical insurance. The records for Child #1,2,3, and 4 did not contain the address for two designated persons to contact in case of an emergency if the parent cannot be reached. Child #1 and 2 were present during the inspection and had been in care for four months. Child #3 was present during the inspection and had been in care for over 1 year.

Within seven business days of a child's first day of attendance, the provider shall obtain from the parent verification of the identity and age of the child. The information was not available for Child #1, 2, 3, and 4. Child #1, 2, and 4 have been attending for four months. Child #3 has been attending for more than one year.

The provider shall review the emergency plan at least annually and update the
plan as needed. The provider shall document in writing each review and update to
the emergency plan. The review of the emergency plan was overdue by more than 6 months.
