Inspector Notes
An announced initial inspection was conducted on May 2, 2024, from 9:25am-11:45 am. This is a transfer of providers requiring an initial inspection, the assistant of the current license has now applied to be the provider. At the time of entrance, ( 5 ) children ( 14 ) points were in care with ( 2 ) caregivers present. The inspector reviewed 4 caregiver/household member records, 3 cribs, emergency supplies and 1 medication. The entire one level house and outside- playground was inspected. The applicant will obtain new records for each of the children enrolled. Children were observed engaged in play time, toileting and handwashing. Areas of non-compliance are identified on the violation notice.
The provider must send documentation to the LI of the results from the central registry before a license can be recommended.

Violations
1Evidence: The Little Tykes climber outside was found with several areas cracked. One are at the top of the tunnel, the entrance to the tunnel and the side of the climber. These cracks are a potential hazard to the children in care.
