Inspector Notes

An unannounced, on-site monitoring inspection was completed on July 24, 2024 as a part of the conditional licensure period. The on-site inspection began at 9:10 am and ended at 10:15 am.The inspector reviewed compliance in the areas listed above. There were (3) children present and (2) caregivers; the point total was (9). The inspector reviewed (4) children?s records, the caregiver and household records are current. This inspection included :
? document review (children records, fire drills, authorization forms),
? tour of the facility,
? interviews,
? observations (children sleeping, being fed and playing dress up)

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Standard 8VAC20-800-240-A
Based on observation, furnishings are not maintained in a clean safe and operable condition.
Evidence: Two children blue chairs were found with cracks in the seat.
Plan of Correction: The chairs will be trashed.
Standard 8VAC20-800-480-E
Based on observation, play equipment used by the children was found cracked exposing sharp edges.
Evidence: A car track toy was found on the toy shelf cracked, exposing sharp edges.
Plan of Correction: The broken car track will be trashed.
Standard 8VAC20-800-50-A
Based on a review of attendance, the provider is not maintaining a written record of attendance each day.
Evidence: There was no documentation of attendance for today or each day for children in care.
Plan of Correction: The provider will take daily attendance.
Standard 8VAC20-800-540-A
Based on observation an infant was observed in a infant carrier seat and high chair without the use of the safety strap.
Evidence: Child # 1, 11 months was first observed in a carrier seat without the safety strap securely fastened. The inspector asked the caregiver to strap in the infant, which she did. Then when the infant woke , the infant was moved to the high chair. The safety strap was not fastened for the infant in the high chair either.
Plan of Correction: Caregivers will always use the safety straps to securely fastened the children in the infant seats and high chairs.
Standard 8VAC20-800-90-A
Based on a review of children records, the provider has not obtained documentation of child's immunization record before a child attends the family day home.
Evidence: Child #1, date of first attendance 7/15/2024 does not have documentation of immunization record prior to fist day of attendance. The child was observed in care this date.
Plan of Correction: The provider will obtain the immunization records from the parent.
